Лабораторна діагностика хронічної хвороби нирок у свійських собак

Анотація

Дослідження проведено на собаках із підтвердженою хронічною хворобою нирок шляхом клінічного обстеження, гематологічного, біохімічного аналізу крові та лабораторного дослідження сечі. Встановлено, що розвиток захворювання супроводжується суттєвими порушеннями показників крові та сечі, які прогресують відповідно до ступеня ураження ниркової тканини. На ІІІ стадії захворювання у собак виявлено зниження кількості еритроцитів (Р<0,01), вмісту гемоглобіну (Р<0,001), величини гематокриту (Р<0,001) та кількості тромбоцитів (Р<0,001), що свідчило про розвиток анемічного синдрому. Одночасно реєстрували підвищення кількості лейкоцитів (Р<0,001) та зменшення частки лімфоцитів (Р<0,05), що вказувало на наявність супутніх запальних процесів та імунологічних порушень. Біохімічними дослідженнями встановлено зниження вмісту альбумінів (Р<0,01), значне підвищення вмісту симетричного диметиларгініну (Р<0,001), сечовини (Р<0,001), креатиніну (Р<0,001) та неорганічного фосфору (Р<0,001), що характеризувало порушення фільтраційної здатності нирок і накопичення продуктів азотистого обміну. Під час дослідження сечі у собак на ІІІ стадії захворювання виявлено зниження її відносної густини (Р<0,001), появу протеїну (Р<0,001) та збільшення кількості лейкоцитів в осаді. На четвертій стадії хронічної хвороби нирок усі виявлені зміни були більш вираженими. У собак на IV стадії ХХН реєстрували тяжку анемію, виражену тромбоцитопенію та лімфоцитопенію на тлі лейкоцитозу. Концентрація показників, що характеризують функціональний стан нирок, зростала у декілька разів порівняно з показниками клінічно здорових тварин, що свідчило про значне пригнічення або втрату екскреторної функції нирок. Встановлено розвиток гіпопротеїнемії та гіпоальбумінемії, що пов’язано з прогресуючим ушкодженням нефронів і втратою білків. Для сечі були характерними різке зниження відносної густини, виражена протеїнурія, збільшення кількості лейкоцитів та, в окремих випадках, анурія. Отримані результати підтверджують, що комплексне визначення показників крові та сечі є інформативним підходом для оцінки функціонального стану нирок, визначення стадії патологічного процесу, моніторингу перебігу захворювання та прогнозування його наслідків у собак із хронічною хворобою нирок.
